Thank you for entrusting Grand Rapids Women's Health with your care.  We look forward to serving your needs. We strive for complete transparency in our office policies and insurance practices, and offer information on both for new patients.

Your First Visit

We want to make sure you enjoy a smooth and stress-free visit with us. Please arrive at our office at least 30 minutes early to park and register with a member of our registration team. Remember to bring your parking ticket with you and we will validate it.

In preparation for your first visit, please fill out and bring the New Patient Packet.  If you did not receive the packet in the mail, you may download patient forms here.

Your medical records are very helpful to your physician, and as such are essential on your first visit.  If any lab work is needed, please be prepared to notify us if your insurance company has special laboratory preferences so that you receive the highest benefit available for these services.

Our office does our best to keep your scheduled appointments, however our physicians may be called out of the office for deliveries or emergency surgery at any time. We do our best to keep you informed of this and to accommodate you in any way we are able.

Established Patients

Please arrive 15 minutes before your appointment is scheduled to begin so that we may verify that all of your information is up-to-date.  Remember to bring your insurance card to every visit.

Office Policies & Insurance

Children under the age of 18 must have a parent with them during each office visit.

Payment is due at the time of service unless other arrangements are approved in advance. If you have a co-pay with your health insurance, please be prepared to pay at the time of your visit.  For your convenience, we accept Visa, Mastercard, and Discover.  Should you have any questions regarding our fees or your insurance coverage, please call our billing team at (616) 588-1100 before the day of your appointment.  Please make sure to bring your most recent health insurance card to every visit.  If no insurance card is present or on file, you will be responsible for payment at time of service.
